Maybe he's permanent camp counselor at that diabetic camp he used to

He's out there somewhere with Gia, who went off to law school after about
one semester of college and has never been seen again.  And Elizabeth's
brother, Dr. "Elf," ...who is apparently always practicing on a different
floor at GH than the one where all the action is happening!   LOL
