I think it was residual imagery from his death. When Ana Lucia appeared to
Eko, she appeared normal at first, then with blood on her stomach and
other
areas. Boone has appeared in a bloodied state to Locke both before and
after he died. Eko's brother Yemi appeared to have blood on him sometimes.
The fact is, Horace had a nosebleed from the gas Ben and the original
inhabitants used during the purge. The last time we saw him, he had blood
coming from his nose, and that's how he appears now.
